Welcome to the Your Happy Days nomination page. You are one step away from nominating that special deserving person to be in the running of having their dream granted for the day!

Below is a reminder of the four different categories and the eligibility criteria. Please ensure that you select the correct category for your nominee. Below the categories is the nomination form; please fill in all of the required details (your details as the nominator and your nominees details). In addition to the details, you will need to write why your nominee deserves something special and details of what their dream Happy Day would be.

We also provide the option for you to download the nomination form for you to complete in your own time. You can email the completed form to nominations@yourhappydays.org, click the download button to download the form.
